Pre-Registration with the practice

We continue to welcome all new patients to our surgery ~

Please read and print all the information out for completion and return all 3 forms to the surgery along with proof of photo ID and current address confirmation eg: utility bill

New Patient Registration Questionnaire : this needs to be completed and returned with all registration forms

Patient Access Consent Form : completion of this consent form will allow you to have part access to your records until we receive your medical records and then you will have full access, please note this process may take a while, please be patient - PATIENT ACCESS for patients over 16 years of age.

It can take a considerable time for us to receive your medical records from your previous GP therefore please complete as much information as possible

Once you have completed all three forms please take in person to reception at the surgery remembering to take your ID and utility bills as confirmation of proof of your address

PLEASE REMEMBER TO BRING IN YOUR ID - THIS IS REQUIRED FOR EACH PERSON WISHING TO REGISTER AT THE PRACTICE AND ALSO FOR PATIENT ACCESS which is our online access service to your medical records  - this service is currently for patients over the age of 16 years old.
