What is a Primary Care Network – PCN?
Primary Care Networks (PCNs) are a model of team-based primary health care delivery. PCNs are groups of doctors working collaboratively with teams of health care professionals, such as nurses, dieticians and pharmacists, and voluntary sector, working together to meet primary health care needs in their communities

Matthew Boyce is a welcome addition to our healthcare team providing First Contact Physiotherapy support to our patients.
Matthew is based in the practice on a Tuesday, Thursday and Friday.

Cheryl Galea is a Social Prescriber Link Worker from Age Concern who is working with the practice to provide support to our patients.

Mental Health Practitioner
Karen Humes is a Mental Health Practitioner working with the practice seeing patients referred from the GPs offering mental health support and guidance in a safe and confidential environment within the practice.
Karen is based in the practice on a Tuesday and Thursday.
All appointments with Karen will be following a GP clinical assessment.
TAPP Programme
Jacob Humes is a Trainee Associate Psychological Practitioner (TAPP) working with the practice to offer support to our patients of all ages who may be experiencing issues around, but not limited to stress, low mood, anxiety, depression, reduced or low motivation, difficulty sleeping and poor general wellbeing.
Jacob is based in the practice on a Tuesday.

Primary Care Network Pharmacist
Victoria Gabbott is a primary care network pharmacist working as part of our practice pharmacy team to carry out structured medication reviews for our patients both within the community and care home settings.
Victoria is in practice on a Monday, Tuesday and Friday.

Jessica Kavanagh is a primary care network nurse associate working as part of our practice nursing team providing patient care for our patients both within the community and care home settings
