SOCIAL PRESCRIBING TEAM

South Ribble Council host a Social Prescribing Team who are there to support you and build confidence to manage your own health and wellbeing and address any other issues that may be impacting your day to day life.

You can speak with a member of their team who will be able to understand if our service could help you, or whether they may be able to put you in touch with other services who are better placed to help.

The support the council provides is free, confidential and personalised support that starts with you and guided by you.

Once you have contacted the council, someone will be in touch with you for a chat to get to know what they can help you with; from there they will sort out an appointment and support going forward.

The council want to support you in a way that works for you, so you will have the choice over whether you wish to have your appointments over the phone or in person.

Social prescriber intervention (sometimes know as community referral) works by providing support for any of the following issues:

  • money worries
  • housing issues
  • isolation / loneliness
  • employment / new skills
  • diet / exercise
  • mental health and wellbeing

How to get in touch

The team is available between 9am and 5pm Monday to Friday.

You can contact them on 01772 625286 or via email to social.prescribingSR@southribble.gov.uk.

Please send them your name, a contact number and email and a brief summary of what support you need.

If the support, you need is urgent or an emergency you should contact the most appropriate service. In any event of an immediate risk to life call 999.
