Paramedic

Andy Wilson is a paramedic working together with the clinical teams to support our exisitng patient services both in the practice and in the community.

Andy works throughout the week and has a mix of face to face and telephone appointments alongside community visits.

Your triage requests may be signposted to our practice paramedic service

First Contact Physiotherapist

We have an in-house musculoskeletal first contact practitioner (or FCP). 

FCP's are trained specialists and can help patients with musculoskeletal issues such as back, neck and joint pain by:

  • assessing and diagnosing issues
  • giving expert advice on how best to manage their conditions
  • referring them to specialist services if necessary

If you contact the practice with back or joint pain, you may be booked in to see our FCP in the first instance rather than a GP. 

We have physiotherapy services based in the practice on a Monday

Nurse Associate

Jessica Kavanagh is a primary care network nurse associate working as part of our practice nursing team providing patient care for our patients both within the community and care home settings

Jessica is in practice every week day.

Primary Care Network Social Prescriber

 

We have an in-house social prescriber attached to the practice. 

Social prescribing (sometimes know as community referral) works for a wide range of people including those:

  • with one or more long term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing (e.g. money or debts issues, domestic abuse, housing, employment)

Suitable patients will be referred by the clinical team and our social prescriber will contact you to arrange an appointment.

Catherine Bradshaw-Annand is our social prescriber who works across all practices offering signposting of services within the community.

Catherine is in the practice on a Monday.

Clinical Pharmacist

Our clinical pharmacist works as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly.  Patients may be offered a structured medication review (SMR).  A structured medication review (SMR) is a patient centred review of all medications, with the aim to optimise treatment and provide better patient outcomes.  

Osman Hajat is our primary care network pharmacist working as part of our practice pharmacy team to carry out structured medication reviews for our patients both within the community and care home settings.

Osman is in our practice each day

Please contact the practice if you have any medication or prescription queries that can't be dealt with by your community pharmacist service.

Social Prescribing Service for Children & Young People aged 5 to 17 years.

We have partnered up with our Primary Care Network (PCN) and Barnardo’s to provide ‘Lancashire Link’, a new Social Prescribing Service for Children & Young People aged 5 to 17 years.
Eligible registered patients can now be referred into this service who can provide assistance with:
*Confidence & low self esteem
*Peer difficulties and community engagement
*Developing resilience
*Family relationships
*Social isolation
*School worries
*Future goal setting
*Worry management
*Coping with stress/mild anxiety
